Steroid types and hair loss: what you need to know

Anabolic steroids have moved beyond the world of competitive bodybuilding. They are now used by gym-goers, amateur athletes and increasingly by young men chasing a specific physique. Yet what is less openly discussed is the effect these substances can have on the hair. At My Hair UK, we speak with many men who notice hair loss during steroid use. The link is often overlooked, dismissed as genetics or stress, but the relationship is well recognised in medical literature and deserves a clearer conversation.

Steroids come in different forms. Testosterone-based steroids such as testosterone enanthate, cypionate and propionate are commonly used to enhance muscle growth. Others, like nandrolone and trenbolone, are derived from testosterone but modified to increase potency. Then there are DHT-based steroids such as stanazolol and drostanolone. These convert to dihydrotestosterone (DHT), the hormone strongly associated with male pattern hair loss. When DHT levels increase at the hair follicle, the follicle gradually shrinks and produces finer hairs until it eventually stops producing hair entirely. Individuals genetically predisposed to androgenetic alopecia are more sensitive to this process, and steroids can accelerate what would otherwise have taken years.

Research backs this association. A study describes how anabolic steroid misuse can trigger or hasten androgenic alopecia in susceptible individuals. Another paper in highlights that exogenous testosterone increases serum DHT, creating a hormonal environment similar to that seen in premature balding. These effects are reversible in some men if steroid use is stopped, but for many the follicles deteriorate beyond recovery. Once miniaturisation has progressed too far, medical treatment or surgical hair restoration may become the only viable options.

Symptoms vary, some notice sudden shedding around the temples or crown. Others describe increased hair on the body while the scalp begins to thin. This paradox of body hair growth and scalp hair loss is typical because scalp follicles are sensitive to DHT, while body hair often grows in response to it. Oral anabolic steroids tend to carry a higher risk because they rapidly influence hormone levels, though injectable forms are not without consequence. The longer the cycle and the higher the dose, the more pronounced the risk.

Many individuals try to counteract the hair loss with finasteride or dutasteride, drugs that block the conversion of testosterone to DHT. They can be effective, and Randomised Controlled Trials  demonstrate their ability to slow or reverse androgenetic alopecia. Yet they are not without side effects, and blocking DHT while continuing high dose steroid use is not always medically straightforward. This is why medical advice is essential rather than self-medication based on online forums.

If hair loss has already developed, some men begin to consider surgical options. At My Hair UK, transplant procedures are commonly used to restore hair in affected regions. how much a transplant cost in the UK has become cheaper in recent years, and for example a transplant of 2000 grafts is typically around £3,699 while 2500 grafts costs approximately £4,099. Beard transplants are fixed at £2,899. These procedures relocate healthy hair from resistant areas such as the back of the scalp to areas affected by steroid-accelerated thinning. Surgery is only appropriate once steroid use has stopped and hormone levels have stabilised, otherwise transplanted hair may also be at risk.

Before booking any consultation with a hair transplant clinic, we recommend speaking to your local GP for an overall health check. Steroid use can affect liver function, cholesterol levels, blood pressure and mental health. A GP can arrange blood tests, discuss safe cessation if needed, and assess whether any underlying endocrine problems are present. It is not a barrier to seeking treatment, but a sensible first step to ensure your health is stable and surgery would be safe. Early intervention also increases the likelihood of preserving existing hair through medical treatment without needing surgery at all.

Hair loss linked to steroid use raises wider questions about expectations placed on appearance, pressures in gyms and the lack of regulation around performance enhancing drugs. If you are noticing hair changes and have used steroids, do not dismiss it or wait for it to worsen. Speak to a doctor, consider stopping steroid use and seek medical advice. At My Hair UK, we see the emotional weight hair loss can carry, and while transplantation can restore hair, protecting what you still have should be the first priority.
